Please fill out the intake form after booking/ before your appointment. $15-$20 is taken off the regular rate.

Accidents happen, if you are in a motor vehicle accident, I am here to support you in your recovery. After scheduling this 60-minute session, you will receive an email with a form asking for your claim #, date of injury, insurance info, and a link to add your referral for massage. Please bring your insurance, ID card, and referral to your first visit so I can have the information I need to successfully bill insurance.
